You’re an Australian resident or Medicare card holder

Your condition/s are present or are likely to be present for 6 months or longer. (e.g. back & knee pain, shoulder injury, post surgery, or tendinopathy)

You need ongoing care from more than one health professional (e.g. GP + physio)
